The Population Health Manager will support the Administrator of Population Health with management of the day-to-day operations of the Population Health Department including providing leadership, coordination, and oversight of population health services within the County, including disease prevention, health promotion, and emergency response. This is a salaried, exempt position.
Under the direction of the Administrator of Population Health provides oversight of population health initiatives and strategies to meet the current and projected needs of Delaware County and the Population Health Division.
Serves as a member of the Leadership Team for the Population Health Division. Supports the Administrator of Population Health with providing overall direction and oversight of population health services and public health emergency preparedness and response.
Coordinates strategic planning initiatives in accordance with the Division's mission, vision, and values.
Supports the Administrator of Population Health with the implementation and oversight of policies, procedures, objectives, and evaluation criteria for disease prevention and health promotion programs; and monitors and evaluates ongoing programs to assess program effectiveness, quality of service, and cost justification.
Supervises Community Health Planners and Community Health Workers and other professional, technical, and administrative personnel within the Population Health Division. Identifies current and future staff development needs and training opportunities.
Develops strong community partnerships to address population health issues including health equity.
Coordinates activities and collaborates with other County departments and divisions, state agencies, and community service groups; represents the Department on community and state advisory and planning boards and participates in various Department and community committees or special projects, particularly as they relate to disease prevention and health promotion
Promotes public health and equity within the community
Takes personal responsibility to provide exceptional customer service to promote and maintain a positive Public Health image, constructive working environment, and foster pride and professionalism in the workplace and community
Performs other duties as required
Supervisory experience required; population health and/or emergency response experience preferred.
Demonstrated experience effectively leading and directing staff and/or volunteers.
Project management experience, preferably in a public health setting.
Bachelor’s degree from an accredited college or university required, Master’s degree preferred.
Strong problem-solving skills and ability to create innovative solutions to the County's toughest health concerns
Good organizational skills with a key attention to detail for coordinating community health initiatives, including budgeting and scheduling.
Skilled communicator with the interpersonal and leadership abilities needed to supervise various health and emergency response professionals
Quick decision-making and critical thinking skills
Ability to maintain and handle confidential information
Ability to follow protocol, procedures, and established guidelines
Excellent communication skills
A strong work ethic and ability to manage a demanding workload
Strong relationship-building skills
Up to date COVID 19 vaccination is required
Valid driver’s license and ability to pass a background check
