Summary

The Community Health Planner (CHP) promotes individual and population health through health promotion and education initiatives and health behavior-change programs in a variety of settings for the diverse populations represented in Delaware County.


Essential Duties

Builds relationships and implements strategies with community-based organizations engaged in health education and health-related services to impact the health and preparedness of residents in Delaware County.

Provides coordination assistance for agencies and community partners working on the Community Health Assessment (CHA) and the Community Health Improvement Plan (CHIP).

Develop and implements work plans to address goals, objectives, and strategies for improving health priorities identified through the CHIP and DCHD, and monitors progress.

Develops and implements strategies, initiatives, and activities for individuals, families, and population groups that promote health and prevent disease, injuries, and disabilities in settings including, but not limited to, homes, community organizations, businesses, schools, and the community in general to improve community resilience.

Supports public health emergency preparedness by emphasizing collaborative engagement and actions that directly affect the social determinants of health inequities in communities. Supports Public Health Emergency Preparedness, including Medical Countermeasures (MCM) with planning, training, and exercises.

During emergency activations, supports public health emergency response and recovery activities. This includes operations support in the field and/or at the public health emergency operations center.

Prepares and disseminates health education materials and information that are accurate, culturally appropriate and educationally sound, including, but not limited to, brochures, press releases, newsletters and postings on social media.

Presents submissions for posting on the Department's website and other social media platforms.

Gathers, organizes, interprets, and maintains data for program monitoring, reporting, evaluation, and improvement.

Plans and conducts workshops, presentations, and training programs for individuals and groups.

Evaluates outcomes, effectiveness of plans, and makes changes as necessary.

Assists with writing reports and presentations and submitting reports in an accurate and timely manner.

Employs data-driven decision-making to determine pertinent program priorities, goals, and objectives.

Searches for funding opportunities and assists in the development of grant proposals, work plans, goals and objectives related to existing and future programs/projects.

Represents the Department by serving on and leading various committees, coalitions, and community alliances related to program areas or as assigned by manager or designee.

Performs data entry and retrieval functions utilizing a variety of electronic platforms.

Promotes and facilitates the incorporation of Departmental core values, vision, mission, and strategic initiatives into daily service delivery.

Develops a working knowledge of other Department services and programs and provides information to the public as appropriate.

Prepares accurately and maintains various records, reports, correspondence, and other Departmental documents including mileage reports and time sheets.

Attends community outreach events to promote education and services within the health department.

Attends trainings, courses, seminars, and conferences as approved by Health Director or designee, to maintain knowledge of current trends and to develop skills necessary to assure duties are performed satisfactorily.

Adheres to all HIPAA and Department confidentiality and privacy policies and practices.

Establishes and maintains effective working relationships with coworkers, stakeholders, governmental officials, and the general public.

Assists in training and orientation of new employees.

Performs other duties, as assigned.


Qualifications

Bachelor's degree in Public Health, Communications, Marketing, or a related field.

Knowledge of the core functions and essential services of public health.

Knowledge of education program planning, implementation, and evaluation.

Knowledge of disease prevention, health promotion, lifestyle risk reduction education, and public health research methods.

Exceptional writing and interpersonal communication skills are essential.

Proficient in Microsoft Office computer applications. Experience/training utilizing database, statistical, and graphics software desired.

Ability to make professional decisions within the framework of Department policy, and perform all duties with efficiency, thoroughness, accuracy, and attention to detail.

Ability to assess, organize, and prioritize work assignments and meet deadlines.

Ability to understand and communicate effectively utilizing the English language both verbally and in writing, including use of proper grammar, vocabulary, spelling, and punctuation. Bilingual skills a plus.

Ability to effectively present information, data, and statistics both verbally and in written communications.

Ability to understand and follow verbal and written instructions.

Ability to communicate, deal effectively and courteously with individuals of all ages and from a wide range of social and economic backgrounds, including those who may be irate.

Ability to conform to scheduling demands that may include weekends, overtime, emergency call-in, and varied days, hours, and shifts.

Ability to work independently with a minimum level of supervision after completing training and probationary period.

Valid driver's license and ability to pass a background check.

Ability to interact appropriately and effectively with a wide range of persons, and work as part of a team.

Ability to maintain and handle confidential information.

Ability to follow protocol, procedures, and established guidelines.

Ability to be flexible and adapt to changing circumstances and needs.

A strong work ethic and ability to manage a demanding and changing workload.

Strong relationship-building skills.


Preferred Skills, Knowledge & Experience:

Strong understanding of public health principles, practices, and methodologies.

Experience in designing, implementing, and evaluating community health programs and initiatives.


Computer Skills

Proficiency in Microsoft Applications (Office, Word, Excel, PowerPoint, Outlook, Teams).


Physical Demands

While performing the duties of this position, the employee is frequently required to stand, walk, utilize a keyboard, sit, bend at the waist, and talk or hear. Occasionally, the employee will need to reach or work with arms above shoulder height, kneel, stoop, drive, crouch or squat, crawl, drive, climb stairs, twist or rotate at the waist while working; push, lift or carry items.

Visual Requirements: Near visual acuity.


Work Environment

Office settings, and on-site visits to various fieldwork locations.


Other

Evenings and weekends as needed, expectation is one evening and one weekend per month.


* The selected candidate will be required to obtain, at their own expense, 3 forms of clearance, including PA State Police Criminal History Record Check; PA Child Abuse History; and FBI Criminal History Background Check including finger printing prior to employment.

** Delaware County Health Department requires all employees be up to date for their COVID-19 vaccination per CDC guidelines. Proof of vaccination must be submitted before a candidate's start date if hired.
