2 forms needed for Lifetime license

Is your Dog Tattooed or has a Microchip? You are able to get a lifetime license.

Owner fills out "Lifetime Dog License Application"

Portions of the "Permanent Identification Verification Form" must be filled out by the vet

THE VETERINARIAN MUST FILL OUT THE FOLLOWING:

  • Microchip number
  • Write the name of the person chipping or scanning & circle “implanting or scanning”
  • Fill out “Veterinarian Practice number” (Vet’s license number)
  • The Doctor must sign and date the form.

The owner can fill out the rest of the information.
Prices are found on the Lifetime Application and the check gets made out to “Delaware County Treasurer”.

The owner must either mail or bring the forms & payment to:

FORMS REQUIRED TO APPLY FOR A LIFETIME DOG LICENSE

LIFETIME DOG LICENSE APPLICATION (Completed and signed by the owner)

PERMANENT IDENTIFICATION VERIFICATION FORM (Completed and signed by the vet and signed by the owner)


NEW MICROCHIP

Microchip implanted by Veterinarian

The dog owner obtains the Lifetime Application and Permanent Identification Verification form from either the Treasurer’s Office, local Veterinarian participating in the program or Treasurer’s Website.

The dog owner has the microchip implanted by a licensed veterinarian or a licensed kennel owner.

The dog owner completes the Lifetime Application.

The Permanent Identification Verification Form MUST be completed and signed by the veterinarian/person implanting the microchip, including the Veterinarian BV number.

The dog owner may either mail or bring the completed forms (Lifetime Application AND Permanent Identification Verification Form) and applicable fee to the Treasurer’s Office. Checks should be made payable to the Delaware County Treasurer.

The Treasurer’s Office issues the Lifetime License paperwork and tag.


PREVIOUSLY MICROCHIPPED

Microchip is Scanned to confirm owner information

The dog owner obtains the Lifetime Application and Permanent Identification Verification form from either the Treasurer’s Office, local Veterinarian participating in the program.

The dog owner has the dog scanned by a licensed veterinarian or licensed kennel owner to confirm the microchip information is correct.

The Permanent Identification Verification Form is completed and signed by the licensed veterinarian or kennel owner who performed the scan, including the BV number. This form is also signed by the owner.

The dog owner may either mail or bring the completed forms and applicable fee to the Treasurer’s Office. Checks should be made payable to the Delaware County Treasurer.

The Treasurer’s Office issues the Lifetime License paperwork and tag.
