The Division of Population Health coordinates with community partners, agencies, and healthcare institutions to address “upstream” social determinants of health that impact individual and community health. Critical to effectiveness is the use of public health data that allows for a close look at local communities to identify disparities and health concerns.

Some key services provided by the Division of Population Health include Community Health Assessment; health education and resources for chronic disease prevention; pandemic emergency preparedness, (including COVID response), the Delaware County Health Department Wellness Call Center, and health equity promotion.
